India produces enough food grains to feed its population and even maintains surplus buffer stocks, yet it carries one of the heaviest burdens of malnutrition in the world. This contradiction sits at the heart of the country’s food security challenge. For decades, the goal was simple: produce enough rice and wheat so that no one starves. That goal has largely been met. But a stomach filled with cereals is not the same as a body that is well nourished. When food security is measured only by the quantity of grain on a plate, the deeper question of quality, of whether people are actually getting the proteins, vitamins, and minerals they need, gets quietly pushed aside.

What food security really means

Food security is commonly understood through four pillars: availability, access, utilisation, and stability. Availability means enough food is physically present in a region. Access means households can actually obtain it, whether by growing it or affording to buy it. Utilisation refers to how the body absorbs and uses nutrients, which depends on diet quality, clean water, and health. Stability means all of this holds steady over time, even during droughts or price shocks.

The trouble is that policy has historically focused almost entirely on the first pillar. The term itself was first used in development discussions of the 1960s and 1970s, when the fear of famine dominated thinking and the response was to grow more grain to ward off mass starvation. Producing more cereals was the rational priority at the time. But once that battle was largely won, the definition of “secure” did not expand quickly enough to include nutrition.

The shift from food security to food grain security

A recurring criticism in academic and policy circles is that food security in India has effectively been narrowed down to food grain security. The Green Revolution, which transformed the country from a food-deficit nation into a self-sufficient one, was built on rice and wheat. Government procurement, the Minimum Support Price system, and the Public Distribution System were all designed around these two crops.

This cereal-heavy model has had an unintended consequence. By prioritising staple production through favourable subsidies for rice and wheat, agricultural policy shaped a food system insufficient to meet the population’s micronutrient needs. Pulses, which are critical for protein in largely vegetarian diets, became relatively expensive, and consumption fell. Vegetable intake, essential for vitamins and minerals, stayed low. The result is a diet that delivers calories but not balance.

Hidden hunger: when calories are not enough

This brings us to a concept that explains India’s paradox better than almost any other: hidden hunger. Hidden hunger refers to a situation where people consume adequate calories but remain deficient in essential micronutrients like iron, zinc, vitamin A, and iodine. A person can feel full and still be malnourished. The hunger is “hidden” because it does not look like the emaciation we associate with famine; it shows up instead as anaemia, weakened immunity, stunted growth, and poor cognitive development.

The roots of this run deep. Indian diets remain heavily cereal-based, with the majority of dietary energy coming from staples rather than protein-rich or fresh foods. Even households that can afford better often eat monotonous diets because of habit and a lack of nutritional awareness, while aggressive marketing of cheap ultra-processed foods pushes consumption toward calorie-dense but nutrient-poor options.

The affordability barrier

For poorer households, the problem is not just awareness but cost. Fresh, nutrient-dense foods such as fruits, pulses, and animal proteins are expensive, and food price spikes make them even harder to reach. An FAO assessment found that a striking 74.1 per cent of Indians cannot afford a healthy diet. Even where food grain security exists, a balanced plate remains out of reach for a large share of the population.

The numbers behind the crisis

The scale of malnutrition becomes clear when we look at national survey data. According to the National Family Health Survey (NFHS-5, 2019-21), 35.5 per cent of children under five are stunted, 19.3 per cent are wasted, and 32.1 per cent are underweight. Stunting reflects long-term undernutrition, while wasting indicates acute, recent undernutrition. These figures are among the highest globally.

Anaemia tells an equally worrying story. The share of children aged six to fifty-nine months who are anaemic rose from 58.6 per cent to 67.1 per cent between NFHS-4 and NFHS-5, and anaemia among young women also climbed. Anaemia is the clearest signal of hidden hunger, because it stems directly from a lack of dietary iron rather than a lack of food.

A double burden of malnutrition

India is not just dealing with undernutrition. It is simultaneously facing rising obesity, a phenomenon known as the double burden of malnutrition. Even among children under five, the share who are overweight increased to 3.4 per cent, up from 2.1 per cent, suggesting that even when children do not go hungry, their diets are dominated by junk food and lack micronutrients. Adult obesity has risen as well. This means policy now has to fight deficiency and excess at the same time, often within the same household.

Research using NFHS-5 data has even documented mother-child pairs experiencing this overlap, where overweight mothers have anaemic or stunted children in the same home. It is a vivid illustration that the problem is about diet quality, not simply the absence of food.

How India measures up internationally

The Global Hunger Index (GHI) offers a comparative lens. Published annually by Concern Worldwide and Welthungerhilfe, it scores countries using four indicators: undernourishment, child stunting, child wasting, and child mortality. In the 2025 Global Hunger Index, India ranked 102nd out of 123 countries with a score of 25.8, placing it in the “serious” category.

The previous year was similar. The 2024 GHI placed India 105th out of 127 countries with a score of 27.3, again “serious,” with the report noting India’s high rate of child wasting. South Asian neighbours including Bangladesh, Nepal, and Sri Lanka have consistently scored better, landing in the “moderate” category. The Indian government has at times disputed the GHI’s methodology, arguing that indicators based partly on small survey samples can misrepresent ground realities. That debate is worth noting, but even India’s own NFHS data confirms that malnutrition remains a serious and structural problem.

What the policy response looks like

India has built one of the world’s largest food safety nets. The National Food Security Act (NFSA), 2013 marked an important shift, moving food security from a welfare scheme to a legal right. It extended the Targeted Public Distribution System to roughly two-thirds of the population, providing subsidised grain to around 80 crore people. Notably, the Act itself was framed around “food and nutritional security” using a life-cycle approach, with provisions for pregnant women, lactating mothers, and children.

From grain delivery to nutrition delivery

The challenge is that the delivery machinery was designed for grain, and shifting it toward nutrition is slow work. Several recent initiatives try to bridge this gap. Rice fortification, which adds iron, folic acid, and vitamin B12 to rice distributed through government schemes, is one of the most significant, given that the government distributes over 300 lakh tonnes of rice under NFSA provisions. POSHAN Abhiyaan, now folded into Mission Poshan 2.0, uses real-time monitoring and community mobilisation to tackle stunting and anaemia among children and women.

Schemes like the Mid-Day Meal programme, rebranded as PM POSHAN, and the Integrated Child Development Services (ICDS) target the most vulnerable groups directly. The push to promote millets, nutrient-rich and climate-resilient grains, reflects a recognition that the cereal basket itself needs to diversify beyond rice and wheat.

Why fixing this matters beyond health

Malnutrition is not only a health issue; it is a development issue. Stunted children tend to have weaker cognitive development, which affects learning and later earning capacity. Anaemia reduces productivity in the workforce. The Economic Survey 2024-25 itself flagged that focusing on food availability alone has masked a deeper crisis of nutritional insecurity that threatens long-term economic growth. A nation cannot fully realise its demographic dividend if a large share of its young people are nutritionally disadvantaged from infancy.

The way forward, experts increasingly argue, is to redesign the food system around nutrition rather than calories. This means revisiting procurement and the Minimum Support Price to encourage pulses, vegetables, and biofortified crops; widening the Public Distribution System basket beyond rice and wheat; and treating diet quality as a core measure of food security. Linking agriculture directly to health outcomes is the structural correction that the cereal-centric model has long delayed.

The takeaway

India has largely solved the problem of growing enough food. The harder, unfinished task is ensuring that the food people eat actually nourishes them. As long as food security is treated as a question of how much grain reaches a household, hidden hunger will persist behind full stomachs. The real goal is nutritional security: a diet that is not only sufficient in quantity but rich in quality, reaching every child and every mother across the country.
